12 Day Functional Restoration Programme

The 12 Day programme is a group based programme delivered over an eleven day period.  This is aimed at individuals with persistent musculoskeletal pain in which they had difficulty controlling, high bio-psychosocial scores, altered pain behaviour patterns and who have either stopped work or are struggling to work and have altered psychology.   

Suitable candidates will probably have undergone multiple treatments including surgery, and have been given different diagnosis.  This programme provides full education, psychological and social support, intense and progressive exercise, stress management and relaxation techniques to facilitate a return to functioning.  This programme is tailored to suit individuals who require both non- residential and residential programmes, although it is recommended that individuals attend a residential programme to gain optimum results.

Duration

Pre-treatment, 75 hours over twelve days.  Monday to Friday for 2 weeks and then discharged for 4-6 weeks and then followed by a completion day.  

Assessment and outcome measures

Self efficacy questionnaire, pain scores to include SF 36. CSQ, speed walk, stairs, sit to stand tests. 

Follow up

We would like to see patients after a 6 and 12 month period post discharge from a programme.

The programmes are delivered in our purpose built centres, using the facilities of local 4* Hotel with a superior leisure complex and accommodation.

Suitability

Persons suitable for treatment 12 Day FRP will probably have undergone multiple treatments and been given different diagnosis.  They may have had surgery and suffering from musculoskeletal pain that has not responded to single treatments of physiotherapy or other specialist therapy/treatment. They will be experiencing some limitation of function in terms work and leisure. They will usually have some concerns about the cause of pain, the optimal treatment and possible deterioration in the future. Examination will reveal sub-optimal levels of fitness and flexibility.

Selection criteria

Interdisciplinary clinical assessment plus range of measures. Duration of pain is not important as some attendees will have had pain for less than 12 weeks. Most will be having difficulties at work.

Description

Group programme of up to 10 persons.

The aim of the programme is to allow people with severe musculoskeletal pain to understand the mechanisms of pain, overcome fear of damage, improve their posture, range of movement, strength and fitness, to manage their pain effectively. Specifically:

  • Education into the cause and nature of pain with particular emphasis on the distinction between pain and damage.
  • Aim to teach self management techniques.
  • Aim to in-corporate an individual exercise programme to improve strength, and range of movement and general fitness, with particular emphasis on spinal stability.
  • Aim to teach relaxation techniques to improve sleep, stress and pain.
  • Aim to the development of planned and paced return to work.
  • Introduction to cognitive therapy to explore the links between beliefs, fears, thoughts, and subsequently mood and pain, with a focus on stress management techniques and relaxation.
  • General health education with introduction to long term fitness maintenance.
